Application Evaluation Criteria

General Application Form

Application Submitted (References= 20% of overall score)

Business and contact information.

• References:  must have at least 5, UASG may be open, if your 3M rep is unknown, please substitute another end user, etc.
• Proof of liability - ($1,000,000 aggregate).
• Proof of worker’s compensation insurance.
• Photos of work completed in last six months.
• Proof of being in business more than three years.
• Credit check – no flag raised if current or less than 60 days.
  
• Application fee of $500.00

References

Reference contact by fax or email.

• If no response, second fax or email is made.
• If no ultimate response, zero scored for that reference.
• Calls made to other members inquiring about applicant.
• Calls made to 3M representatives.

Interview / Observation

40% of overall score

• Reply made to applicant confirming date.
• Confirm installation under way during interview scheduled date.
• Visit applicant and observe general business practices.
• Specified in predetermined interview format.
• Verify employment of installers.
• View payroll reports or similar information.
• Observe installation and record info via interview format.
• General discussion questions.
• Assemble data and notes, forward to Certification Committee Chairman

Certification Committee Meeting

• Combines reference and interview data.
• Forward information to committee and discuss/vote.
• Notification to applicant of denial or continuation of application.
• Schedule testing session.

Testing Session

40% of overall score

• One or two owners/employees of applicant company are tested.
• Hand-on testing with common films and substrates.
• Written testing on common films and techniques.
• Pre-defined scoring system used.

Final Scoring

• Performed at testing session.
• Three components combined, 80% score or better is approved.
• Applicants review information, files forwarded to Secretary/Treasurer.
• If overall score is less that 80%, then application is denied.
• Opportunity to re-apply in one calendar year.
• Additional application fee of $500.00 required.
• Start screening process over.

Ongoing Requirements

• Maintain ethical standards.
• At least one installer that has passed testing session must always be employed.
• Submit to spot check(s).
• Predetermined spot check evaluation format.
• Interview and installation observed.
• If score of less than 70% and/or key criteria not met (insurance, employed attendee, etc.), then full review/vote by Certification Committee.

Committee Recommendation For Pass/Fail/Probation/Revocation.

• Determine time to comply.
• Spot check again if necessary.
• If 70% or higher score and key criteria met, Certification Committee Chairman may opt for no full committee review.  Forward info to Secretary/Treasurer for filing.
• Resolve warranty issues in a satisfactory manner.
• Disputes or complaints may be filed by fellow members, customers, or 3M.
• Review by Certification Committee if parties not satisfied.
• Certification Committee to recommend action and/or review certification status.  Fees Membership and Certification - $850.00 annually.

Optional Items

• Send additional installers through testing session.
• Members – no charge.
